I am a H1B holder and entered USA in January 2008. My wife and daughter are on H4, they are my dependents. I have received the W2 from my employer and there's no other source income.

I know that I have to apply for ITIN (W-7) for both my wife and daughter.

Question 1: Is it required for my wife and daughter to accompany me to the local IRS office when I go there to apply for their ITIN and file my return at the same time?

Question 2: Which form do I need to use - 1040 or 1040NR?

You will file Form 1040. Attach W7 (ITIN applicaitons) for your wife and child with your tax return and mail. No one needs to go to the IRS office.
