As this is more a tax question I am moving it there so an expert will see it.

Either Form 7701 for partnerships and corporations and Form 4868 for sole proprietorship.

Should be Form 7004 for partnerships or corporations.

I stand corrected; I posted my answer off-the-cuff without checking the form number.
