Hi,

I am filing ITIN ( form w-7 ) for my wife along with my Federal Return. I have few questions.

1. Should I check
e. "spouse of a US citizen/resident alien"
Or
g. "Dependent/spouse of non resident alien holding US visa"
I am on H1 B visa working since 2004. I have lived almost entirely in US from 2001-present..

2. What documents I need to attach along with w-7 ?
3. Do I enter my wife passport information in 6d section of form w7 if I check passport submitted ? What does it mean by "Issue by".

Please let me know.

2. Notarized copies of passport.
3. Yes. Issued by is the country that issued the passport.

Only First & last page of passport... right ? What about Visa page in passport ?

The visa page is NOT required when applying for an ITIN, but adding it does not hurt and could answer questions before they can be asked.
