1099s for unemployment are issued by the local State Department of Labor. So you need to contact them to find out when it will be mailed or if you can get one online.

I removed your personal information. That was VERY foolish to post such information on a public site. With all the info you posted, someone could easily steal your identity. I strongly suggest that you subscribe to a service that will monitor your credit reports for the next year or so to prevent that. At the very least you should contact the credit report agencies and request a freeze on your accounts.
