From your employer
Contact all employers for that year. They are responsible for providing their employees with a W-2 postmarked Jan 31 of that year. It is more likely that you did receive your w-2 form(s) which were subsequently misplaced or lost. You can try contacting a former employer again for a replacement but don't expect a lot of cooperation. You might also try contacting the IRS. The employer certainly sent them a copy. Good Luck!
Call 1-800-829-1040 and ask for a copy of your tax transcipt. The information for the W-2 will be on that transcript, and any competent tax professional can then reprodice the W-2 for you.
You probably can get the same data from your state tax department to get the STATE version of the W-2.
