If you provided daycare out of your home and your clients want to claim the Child Care Credit, you will need to provide them with your name, address, SSN and the amount you received from them for the service you provided.

You need to report this income as a business, filing Form 1040 with Schedules C and SE. This type of tax return is NOT for amateurs, even with tax software.
