    HIPPA violation
Is it against HIPPA to call a patient to update their medical chart with information about past medical conditions, surgeries, and current medications.

Why not question the patient in the office instead of calling them? Why call them in the first place? The patient may or may not wish to reveal this information over the phone to anyone, let alone even his own medical provider.

Its not a HIPAA violation as long as the person is an authorized employee of the health care provider. However it's a bit unusual to do this over the phone and you would be within your rights to refuse or to verify the person calling.

I think it depends on the facility - before I had surgery the Anesthesiologist's office telephoned me to verify and ask questions about my medical history.

I guess my Surgeon didn't share the info but they did have sort of bare bones stuff -

Of course, I didn't believe it really was the Anesthesiologist so I took the number, looked it up and called back.
