Yes you can. I just had that issue. Call the state department of health. They can help you. They sent me the paperwork right away. And I have to pay 15.00 for it. It's a paperwork that you and the father has to sign. Im not sure if you HAVE to but you may need to get a paternity test, They didn't make us do that. :) good luck.
