The end all authority is IANA, which assigns the codes though they are also available elsewhere, like ISO, ITU, etc. For a reference see Explain That, a nice site in Austria that has Quick Reference cards that are quite good. Registration required, two steps and free. The menu is "tricky" though on the left. Part is hidden and will appear when you place the cursor over the menu.
