My dog had 4 puppies 2 days ago, today when I took her for a bathroom break, she wanted me to pet her, so I did but I noticed a big hard lump next to breast, could it be posible that she might still have another puppy inside her.

She could have a pup still, or she could have something like mastitis (an infection of the mammary glands). I'm thinking that you need to take mom to a vet and get her checked out. If she has mastitis, then the pups could get infected as well while they nurse.
