I'm a doctor I suppose to have my permanent residency in canada soon and in the same time I need to have residency in the united states , so I want to know if I can introduce the us residency as an execuse for not staying the required period in canada as an immigrant?and will it be easy for me to take a permit from the canadian health authorities denoting that they lack my speciality so I can get the j1 visa from he united states

In every countries, there are similar regulations regarding physical present in the country. Check with your attorney, how long you can leave Canada without jeopardizing your status. In US, it's recommended not to leave the country longer than 6 months, or your status would be revoked.

The best bet would be wait until you become Canadian citizen, then there would be no restrictions.
