    Hello,

    Could someone tell me where can I get access to the form 5500 for the year 2001 where I could track my old 401K which may have been filed by my ex employer ?

    For blank forms:

    Go to Internal Revenue Service

    Click "More forms and Publications"

    On the "forms and Publications" page click "Previous Years"

    On the "Prior Years forms, Instructions and Publications" page click "2001" (Others may utilize the option to click other years)

    Read the instructions to download the desired form. Form 5500 and instructions are included in the list of links. Note that some of the archived forms might have obsolete submission addresses "printed" on them.

    I don't know how far back the downloadable forms include the ability to type your entries onto the form while displaying them with recent versions of the required, but free, Adobe PDF file reader. Be aware that that feature is for data entry only and doesn't make any mathematical calculations of your tax or any other financial information.

    For copies of your prior returns see the instructions in http://www.irs.gov/pub/irs-pdf/f4506.pdf "Request for Copy of Tax Return" but there's a $39.00 charge and a response may take 60 days.

    If you used a professional tax preparation service that year, they may be able to retrieve a copy from their files, but there may be a delay, since the file may have been transferred to an offline CD-R or a computer server at the chain's corporate office.
