Many of the higher end radios come with the security feature that if the radio loses power, it will not work until you punch in the correct code. This makes stolen radios useless, preventing theft. This can also make non stolen radios useless in the event of something like a battery replacement if you can't find the card with the code on it. I think some of these radios, you can turn the feature off while the radio is still working.
Your first step is to carefully go through all the paper work you have that came when you purchased the car or radio. If it is a factory radio, you could talk to a dealer. They may rescue legitimate car owners. If you bought the car used with the radio in it, and the seller didn't pass on the code, or you have lost it, you may have to buy another radio.
