    Prevailing Wage form A-1-131

    Hello. My question is, in what category (Fed. Tax, FICA, State Tax & SDI) does CA Training Tax, Medicare, FUTA & CA SUI (my accounts) go under when filling out a "Public Works Payroll Reporting Form"?

    There is a spot on the left hand side of the form that says Deductions, contributions, and payments. Under that will be the deductions you have mentioned.

    Also on the bottom of the form it says any other deductions, contributions and or payments whether reqired by prevailing wage determinations must be separately listed. Use extra sheets if necessary.

    I have a copy of one, but couldn't figure out how to attach it.

    your paycheck will have the total hours worked and the gross wage. Divide the gross wage by the total hours worked to get your hourly rate.

    If your gross wage is 1200.00 and your total hours are 80 than your hourly rate is 15.00.

    If you are in construction you may have several listings because you may be paid different wages. They should have total hours and gross wages.

    For example. 30 hours 330.00 (330 divided by 30 is) 11.00 per hour
    38 hours 456.00 (456 divided by 38 is) 12.00 per hour

    12 hours 198.00 (198 divided by 12 is ) 16.50 per hour
    (this would be 11.00 x 1.5 or OT for 11.00 hour)
