    Every court has its own way it likes its documents prepared. They explain how in a book called "Rules of Civil Procedure for XX Court" You can buy a copy of the book at your local college book store and TRY to make your document look exactly like book describes...

    But, you ain't gonnna make it exactly like a lawyer would...

    Therefore, I'd just write a letter. Look, the judge knows you're not a lawyer. If he understands your request, he's obligated to treat it like it was done correctly. As him in your letter to consider it a motion to vacate. Send a copy of your letter to the opposition lawyer. Send them both, certified, return receipt requested.
