First, you need to submit what you did with the adjusting entries and the adjusted trial balance numbers so we can look at what went wrong. (As opposed to just telling you what to do, since we aren't here to do your work for you.) Also, I do not understand why you seem to have two sets of adjusting entries?

Just as a note, since the adjusted trial balance doesn't balance, one of your adjusting entries doesn't balance, or you made an error in going to the adjusted trial balance, like copying something wrong, wrong side, added wrong, etc. See if your adjustments balance first. If not, then find what doesn't balance in there and start with that. If that balances, then it's in the math in going from unadjusted to adjusted.
