My wife and I were not married at the time our daughter was born and I guess some how they didn't put me on the birth certificate. How do I get my name put on it?
If the natural parents were married prior to the child's birth, the father's name can be added through the corrections process.
If the natural parents were married after the child's birth, then the father's name can be added through a legitimation process. This requires the parents to send the Department of Public Health, Division of Vital Records a
* completed Affidavit by Biological Father and Affidavit by Biological Mother forms (obtained from Vital Records);
* a certified copy of their marriage record (which is obtainable from the county clerk's office in the county where the marriage occurred; ; and
* a check or money order payable to the "Department of Public Health" .
You were not added because you the mother of the child did not list you on it, and/or you did not go in and sign it.
Each state has their own requirements to add the fathers name, you will need to contact the dept of vital statistics in your state and find out what your state requires.
By the way, solidsht, I find your poster name rather offensive. Is that the best you could come up with under the circumstances. If you read through our material before you posted your question, you would have seen our site carries some sophistication in its depths along with some really neat, well thought out posters' handles.
