Paid by company that does not file!
I work on a yacht, typically out of the USA. While the yacht(place of business?) has an offshore registry, my pay comes through an American bank, and is split between my offshore bank account and my American bank account as I desire (I put more in the offshore acct.) The company does not file (don't ask me how!), so I have no w-2 with which to file MY taxes. What do I do? As much as I'd like to NOT file taxes, I don't want to pay penalties, either!

I assume that you are a U.S. citizen or a U.S. resident. You should include the income on your tax return as either wages or [self-employed] business income, as appropriate. If you have a financial interest in a foreign bank account, you need to disclose this on Schedule B of Form 1040 and on Form TD F 90-22.1. Any income from the foreign accounts should be included as income on your U.S. tax return.
