    Hi,
My name is Melissa and I want to change my baby's last name from my husband's last name to her father's last name. I need to know the procedure to this because I don't know if my husband will have to sign anything agreeing to her name change or can I just do this on my own? I need to know as soon as possible because I am getting a divorce and want to have everything taken care of as soon as possible and all at once. I plead for some answers as soon as possible. Thank you for your prompt response and your time.

First its not a good idea to piggyback your question on someone else's. This can lead to confusion. You should start a new thread. So I've moved your question to its own thread.

Assuming the baby is a fairly new born, you may be able to just go back to the hospital and ask them how to amend the birth certificate. If you husband is not agreeable to the change, however, the father will need to get a paternity test and he will need to file to amend the certificate.

Since you are planning on a divorce I suggest getting an attorney who can handle the divorce and the name change.
