Contact your local registrar's office that is responsible for birth certificates. They will explain the procedure for adding your husband's name and amending the because.
File a petition for change of name. You can use and attorney for that. You can have an attorney JUST file the paperwork for you. You can call the courthouse and ask them if they can tell you exactly what needs to be submitted. File the papers yourself, and just pay a bit of court cost.
I would try to get a hold of a sample from someone else. Go to your clerk of court office and ask to see a sample of the paperwork. If they can not provide one without a case number, you will have to look court calendars from the past, until you find one that pertains to a change of name. Use the case number to get a look at the file. Copy, or make notes if they won't let you copy, on how the pages are laid out, and how text is written. Go home, make your own court docs.
