I was in OPT from jan 1 2007 to dec 31 2007. I did not earn anything accept 35$ from bank of america as interest. I graduated in may 2007. I lived in newjersey from jan 1 to aug 1 and then moved to chicago, IL.

What forms do I need to apply.
Please help me.

If the $35 in interest is your ONLY income, you have no requirement to file a tax return.

You DO need to download Form 8843 from Internal Revenue Service, file it out, sign it and mail it to the IRS Center in Austin, Texas, but, other than that, you are good to go!
