Social security tax is withheld at 6.2% and medicare at 1.45%. Federal income tax and state income tax withheld depends upon what you have put on your W4.
Some states do not have income tax. Some states have unemployment/disability tax.

Each state has a state-equivalent form for the W-4, and withholding for state taxes depends on what is put on THAT form.
