NAVPERS 1616/24, “Enlisted Performance Evaluation Report"

Wherever you are stationed, and have the authority, they will have the form available for you to fill out. If you are stationed on a ship go to personnel, or division head. If on shore duty go right to admin, show your id they will probably have what you are looking for in that type of format.

However, it is a restricted form for obvious reasons. With that being said, if you are not in the armed forces it might take an Act of Congress to get what you are looking for.
