Hi I have a question concerning my employer. I was asked to sign a medical release form after a incident a work. In my stupidity I signed it without it being filled in, the person told me it would only be for one doctor to get my records for my employer and it would only go to human resources. A few days later I went and asked for a copy of it and she said I couldn't get a copy, I asked to see the form and she said it had been sent to human resources. I've called and they will not call me back. Is there any way to get a copy. I know I was lied to and records were pulled from other places. Thank you.
