I had mu baby 3 months ago and I didn't let the father know I was in labor so I didn't but his name on the birth certificate... know I want to change that how do I solve this problem :confused:... I also want to change her last name to her father last name but I don't know how... please help :(

You can go to the office where you apply for a copy of birth certificates. They should have a form there to fill out. I am quite sure you will have to take your babies dad with you to fill it out.
