My doctor's office will not release a claim to me with all the necessary information such as the diagnosis code and service code (cpt) because of HIPAA ? I have a self insured plan, the doctor is non-par and will not bill it, and I am trying to get myself paid. How can I get this information ? And is it true regarding HIPAA that they can not give this to me ? Help.

I HIGHLY doubt this has anything to do with HIPAA. I can't imagine any reason they wouldn't give that to you except that they may not be prepared themselves to have that information handy.

Are you saying they are out-of-network and that's why they won't submit the claim? If so, contact your insurance company directly and ask what they need. If you're out-of-network you might not get reimbursed at all, but its worth a try.
