1 of the bolt & nut that holds the commode to the floor cannot be tightened as the bolt & nut both turn. Must I remove the commode in order to replace a new bolt & nut or can it be replaced in some other manner?

Sorry but you need to lift the toilet. That bolt should have been caught in a slot in the flange. It may have slipped out or even broken. I'd replace the wax ring while you are at it.
It is possible that if you pull up on the bolt while tightening the nut. the head may catch and you can tighten it up avoiding the mess of removing it completely. Down side to that is it may have rocked enough to mash down the wax ring allowing bad smells and toxic, flammable gas into the house. If it smells bad, you do have to change the ring.
