    Daughter's birth certificate missing
    I need to register my daughter for school on Monday at the latest, but I can't seem to find her birth certificate, I've looked every, I think it might got lost since we have moved three times, any ideas how to get her registered at school without the certificate

    Mine was lost, and I called a hospital (as there are three in this town, and I don't know which one I was born in) and they gave me number to call, but I can't remember who it was at the moment.

    Perhaps someone else who sees your post will know who you have to call. But you can have it re-issued; I'd start with the hospital where she was born.

    Don't you have a certified copy of her birth certificate? You can also use other official documents like her Bank account statements, school reports, etc...

    Call the hospital where your child was born they will direct you to the correct location, usually it is the health department. Not any health department will do. It has to be the one in the location where the birth was given. You should be able to walk in and come out with it in about 15 to 30 minutes.

    You need to go to your local bureau of records and get a copy. You can probably register and bring in the ceritifcate later.

    County Court House (or "County Clerk's Office") of the city she was born ~ that's where any hospital would tell you to call.

    You just show up with your drivers license and they'll probably charge you around $10 for a certified copy.

    Marliy you really need to call the hospital where you gave birth at, to find out where you need to go. For example. In my county it is the health department of the city that you gave birth to the child in. I live in Ohio and my children where born at 2 different hospitals and I had to go to 2 different health departments to pick the birth certificates up. But in Kentucky where I was born , you must get it from the Bureau of Vital Statistics and that is in Frankfort, Kentucky.

    We misfilled my sons birth certificate and social security card.

    I had to go to the county clerks office. I think I paid $15. I also needed my ID and I believe they wanted a record of immunization from the doctors office, which was given to me by the office for free and with a days notice...

    Though I might me mistaken... I might have needed the immunization for the social security card...

    I agree with the post that said you might be able to register and come back with the proper docs... depends on the school. They let me do this for my son with med records.

    So 1) call the school and see if you can bring it late
    2) call the hospital
    3) clerks office if the hospital is no go... there will be a delay in the delivery (a week) if you have to go this route.

    Thank you guys for all you advice it was much appreciated, I phoned the department of home affairs and got a re issue for 3 dollars :)
