The pension or IRA custodian who distributed the RMD will have generated Form 1099-R.

That Form 1099-R was sent to the IRS and you should have gotten a copy as well.

If you did NOT get the 1099-R, contact the custodian and have them send you another Form 1099-R.

You CAN wait and have the IRS add the RMD to your income, but an amendment is probably a better idea, especially if you owe added taxes.
