Hi, I am a grown man, and the first child for my mom. My parents got married years ago but I still goes by my mom's last name. How do I add my father's name to my birth records?

Are you trying to change your last name to his or just add his name to your birth certificate?

Yes I want to add his name to my birth certificate

I know the steps to changing your name... but not on adding it to the certificate. If no one on here gives you an answer call your local courthouse they should be able to direct you in how to handle this or if you can pull them up on the computer it may even have directions on there. I know in my area the health departments handles all of the birth certificate information. But from what I see on here that is not the norm.
