How do I get my membership re-instated/re-activated?

Membership to what?

Union? Talk to the union steward. Most people are leaving unions.

Membership in the Laborers International Union Of North America. I am a journeyman laborer. I have been unavailable and unable to pay my dues for a few years. I've had to do it once before and I remember there being a fee, but I can't remember what I need to do.
