I have a situation in which someone is asking me to help them with their taxes. He has not filed in the last five years and considers himself self employed. He's looking to file this year yet has not kept any records of any of his business dealings nor paid any taxes. What would be this persons best route to go next? Could a CPA or accountant help him? I would like to help him but am not willing to get involved in filing anything that is false.

That someone must prepare complete record of income and expenses. Use bank statements, credit card statements, payment receipts...

This situation is far more common than you think. A year does not go by when someone calls saying they have not filed for 2-8 years and they need to file (my personal record is 17 years).

Any competent tax professional can help your friend prepare his returns.
