    Changing your birth certificate after a legal name change
    I recently legally changed my name. Have all my legal papers, new social secutiy card, drivers license etc. however now my origanl birth cert does not reflect my name change. I was born in up state New York, lived in Nevada where name change was done and now live in Florida. How do I get this changed. I can't even apply for a passport to travel or anything even though its all legal. Please help

    That is probably handled at the county level: Vital Statistics. Give them a call. If they're not the right department, they can direct you to the right one to call.

    And since birth certificates are done in the state you were born ( no national birth certificate) you have to deal with this though the state you were born, some will have a state office, others you have to actually deal with the county you were born. They should have a form, you supply them the court docment of name change and so on.

    BUT,, I will add that they may not change you birth certificate either, an example is a marriage license, your last name changes but your birth certificate does not, to prove ID you provide birth certificate and marriage license. So for a passport you would provide a copy of the name change from the court and the original birth certificate
