The height above the cooking surface should be determined by the largest pot you are going to be filling, unless your biggest pot is over two foot, and then I would keep it at about twelve inches above the pots that you use most.

I agree with Letmetellu... in addition to his advice:
There isn't any given measurement referring to the position of Pot Fillers. From my personal experience, the preference for height and location of Pot Filler varies from household to household.
But in general: yes, the water stub-out for the Filler should be about 12" above the stove . Depending on the type and brand of your Pot Filler, the actual height of the aerator may be about another 2" higher. Since stove is 36" high, than this stub out will be 48" from the floor + 2" .
Now, you have to decide where to position it in relation the width of the stove. Usually it is off centered and over the larger burner. That's where usually is the largest pot positioned.
Also, consider plumbing it to Hot water supply and / or to filtrated water.
