    That kind of formatting advice is hard to give, and is really an ACCOUNTING (vice a tax) question.

    Note that, for 2013, the employee Social Security deduction is also 6.2%.

    First you have to compute all the deductions.

    You will need to do a journal entry for the pay of $1,500 so your debits and credits should equal $1,500 for the pay and the employee deductions, also you will have to do another journal entry to record employer deductions.
