    If your in the U.S. it usually is witnessed by a notary.

    Normally yes , ( each state will have its own rules as to sale of property)

    And the when will make a difference, some years ago, it may not have been required, ( I sold properties some years ago without)

    Is there a issue with the deed, was it recorded after the sale ?
