    Insurance not billed in time
I was seen by the doctor in 2007. They did not bill my insurance or send me a bill until 2009. Now it is too late to bill my insurance. What recourse do I have?

You are responsible for the bill. Usually when you see a doctor and fill out all the initial paperwork, there's a form that you sign agreeing to be responsible for the bill in the event your insurance denies the claim. In short, you are responsible for ensuring that the bill gets paid whether it's out of your pocket or by the insurance company.
