Thanks to everyone who responded. The issue is now resolved and here's what we did to resolve it:

Blue wire connects to 'B' terminal
Yellow wire connects to 'Y' terminal
Green wire connects to...

Can someone confirm that the Blue wire should be connected to the 'W' terminal?

I'm thinking maybe the Blue wire should not go to the 'W' terminal.
