I've been going out with my boyfriend for a long time now, and we had our ups and downs, even broke up a few times, but I could never let him go out of my heart completely: he was with me and supported me even during the times when no one else was there for me.
We met each other when I was 17, and he needed someone to help him get over his previous girlfriend (he's 5.5 yrs older). I fell in love with him and have been ever since. However, I was immature and made many mistakes and he pretended to be someone else just to keep my teenage dream complete. Now I am learning who he really is and sometimes I start feeling very insecure. I still have suspicions that he might be lying to me or that he'd rather be with someone else.

He has a strange way of thinking and believes that it's the girl who makes the choice of who she wants to be with, and guys are just sort of there, I guess. Girls pick guys they want to date with serious intentions (when looking for a long-time partner) and guys just go for whoever looks or performs better.
I can't comprehend that, honestly, but he thinks everything's up to a girl... which is another thing that keeps me worried. Makes me think he doesn't really want to be with me, and that he's with me because I chose him and no one else did. Although when I ask he tells me he loves me and wouldn't be with anyone else but me.

How can I tell if he really loves me and has serious intentions?

Ask him where he sees your relationship going in the next five years? What are his plans for the future, and how do you fit into them. Five years is a long time to be with someone not knowing his feelings for you. You are young, don't waste any more time. If he loved you, you would not be asking anyone else. Search your own heart, you know the answer.

After 5 whole years you start doubting the guy you have been with?? I mean he is still there after all the changes you have gone through, and YOU have doubts?? That's why I ask what is going on in your life that would make YOU insecure now? Why do YOU think HE is lying?

Sounds to me, he has given you complete control, and you don't know what to do with it.
