Can you post a picture. I have never seen a rust hole all the way through a cast iron tub. You can clean it, sand it and paint it with something like Amazon.com: Rust-Oleum 7860519 Tub And Tile Refinishing 2-Part Kit, White: Home Improvement

Almost impossible to successfully "fix" a rusted tub with a hole. It would also look bad. Consider a tub liner or refinish Re-Bath or similar. Cast Iron or steel?

    Hi Juan...

Is this a real hole or just a rusty, pitted surface? Let me know as the fix is different for each issue.

Assuming a hole JUST UNDER THE SPOUT as you said, try some SOIL SEAL.. see image. This product is specifically made to make repairs in cast iron or fill cast iron hub/pipe joints.

Otherwise, an EPOXY PUTTY or J.B WELD may also work well... see other images.

All products are available at the larger home improvement centers and all require that the surface be prepared/cleaned very well so read the can/tube well to make sure you don't need to pick up a grinder wheel, highly abrasive sandpaper, or a scum remover for the best result, OK?

Also note that if you wish to restore the porcelain finish even that can be accomplished using PORC-A-FIX... see image... available in different colors, too! This product is only available online or at some plumbing supply stores (white, almond only).

    Questions? Let me know...

Yep, I'm thinking not actually a hole but a pitted area. May , to get the levelness Brian want the folks putty the area with Marks JJ-B weld and then they topcoat with my Rust-o-leum
