    Lockoutt/tagout
    I went to do a pre op on a piece of equiment with the sanitation supervisor. The supervisor first inspected the machine and put his hand on a rotor. After he did the inspetion I did the same thing. An employee in the area saw what the supervisor did and told him he didn't have it locked out when I was doing my inspection. I put my hand in the stopped machine up to my wrist. Both sanitation supervisor and I did not think to lock out the machine prior to doing the inspection. We both are on supension until they determine what to do. I have no safety citations will I get fired

    That is up to your company, what is the rules on firing someone, is it a union shop ?

    But yes this is a very serious violation and you could be fired for it.

    Whatever the outcome, it's better than losing a hand.
    The supervisor should receive the stiffest penalty. But it is probably going to depend on the past performance for both of you. Where I last worked, the punishment for safety violations was spelled out in a directive and they are probably in writing somewhere if you check and as chuck says union rules could play a part also, if applicable.
