I have a dead beat father who has not been part of my daughters life ever. She is 12 years old and his where about is unknown.I have never received child support. I have recently applied for a passport and can't get it without his permission. I need help getting full custody and taking his name off her birth certificate. As I just found out its not his name any more.

    1. you can not take the fathers name off the birth certificate
    2. you can get full legal and physcial custody by going to court.

    3. what do you mean by not his name anymore ?
Did he go to court and change his name.

Well when he was born they put the last name Bacon on his because and that's the name on my daughters because and now he has his fathers correct last name of Baker, not Bacon. Since that is not his name,I think Bacon should be removed.

You don't know where he is but you know about the name thing?

But that's really not an issue. You need to get full legal custody. To do so you need to go to Family Court and submit a petition. The court will require that you make, at least, a good faith effort to contact him.

I strongly advise getting a attorney to prepare the petition for you.
