    Birth Certificates
I am in Michigan. I am divorced and kept my ex's last name. I am having another baby now and wonder if I can give this child my maiden name as surname. Is this possible?

Yes, its possible. The mother can use any name they wish on the birth certificate. There is no requirement to list the father.

However, the father can bring legal action to have his name listed. Also if you plan on applying for support, you will have to identify the father and he will have to be acknowledged as the legal father.

Scott is correct, you have the legal right to name the baby.

I'm eighteen years old and have lost my birth certificate. I desperately need it in a couple of days. How do I go about getting it and what forms of id are needed?
