    Hampton bay model no.
Where would the model no. Be located? My model is a white 4 bladed fan about 20 years old. It has a controller for light and fan. The bottom of the fan has a chain pull switch and a 2 position slide switch. Inside the bottom there is 2 capacitors, a double 5 & 6 uf and a single 5 uf.

Look for a UPC on the top of the fan housing. Then go to the HD website.
