Ceiling Fan & wall Switch

I am putting in a ceiling fan and wall switch. I have power (black, white, groung) to ceiling box. Running wire to a wall box is not a problem. I would like a wiring diagram to wire this properly.
Light and fan on ceiling fan works but switch won't turn it off?

Took old fan down put new one up wired it the same way,and when power is turned back on the lights and fan work but when you try to turn them off with the switch they won't go off.
