How do you know that? Do you have a volt meter?

The breaker at the disconnect (the breaker outside) usually serves primarily as a means of disconnect. It is normally the same size (amperage) as the breaker in the panel. Breaker do sometimes get weak however.

Breakers do not always move to the full off position when tripped. To insure that a breaker has not tripped, it is best to turn it off and turn it back on. Try that with the breaker in the breaker panel. Try that and then re post.

When you removed the service breaker you must have opened the breaker in your main box, are you certain you closed it?
