    I would talk to your Professor as he or she will be able to tell you the requirements for a cover page at your school. In my classes where I had to write a paper the cover page had to be in a certain format like my name, the topic of the paper, course number, and the name of the Professor. This information should have been given to you as part of your assignment along with other pertinent information you will need to know to write your paper.
