It's more dependent on the state you reside in.
Check here for info on your state.

There are insurance companies that offer IVF treatments, but these treatments are typically found in the group insurance world. Individual policies do not typically cover IVF treatments, but they may cover the diagnosis of a fertility related issue. There are states that mandate more than IVF diagnosis, so coverages (mandated by a state OR are options that can be offered by an employer group).

If you have insurance now, through your employer, it would be prudent for you to obtain a copy of your certificate booklet and review what is covered vs. excluded. If you are on an individual plan, you may want to check your policy regarding this concern or contacting the insurance carrier directly as they should provide you with the answer to your question. Just remember that any written policy contract governs the aspects of covered benefits vs excluded, thus do not rely on verbal communications to carry the same weight.
