motility (within) = 1 hr
actively motile % = 40
sluggishly motile % = 10
non-motile % = 50

abnormal forms % = 20
cells from s.tubules = spermatocytes : 6-8 / hpf
pus cells = 2-4 / hpf

Didn't you ask the doctor that ran this report for you, surely they would be the best qualified to answer you questions..
