You would usually find these in text books but the simpliest format for both could be contained in a Journal book obtainable from a stationer.
A trial balance consists of three columns account name, debt balance, credit balance and there will be column totals. A journal might have four columns date, account name, debit amount, credit amount and there will be a narration.
